Hold On, Is Ozempic Messing With Our Bones Now?

Every time a new miracle drug takes over the culture, a little voice in the back of our minds asks: what’s the catch?

If you’ve been tracking the meteoric rise of GLP-1 medications like Wegovy and Ozempic, you already know they do heavy lifting for weight management and diabetes.

But a fresh batch of data is raising some serious questions, linking these popular medications to a slightly elevated risk of gout and osteoporosis.

Given how heavily these joint and bone issues already impact Black families, we have to wonder: are we trading one health hurdle for another or is this just a minor bump in the road?

Here is what you need to know before your next refill.

Recent research presented at a meeting hosted by the American Academy of Orthopedic Surgeons has sparked discussion about GLP-1 medications, suggesting a possible link to a slightly increased risk of osteoporosis and gout.

According to ABC News, Dr. John Horneff, an orthopedic surgery professor at the University of Pennsylvania and the study’s lead author, began investigating after observing that some patients experienced serious tendon injuries from minor accidents. This led him to dig deeper into whether these popular weight-loss drugs could affect bones and connective tissues in general.

While these medications are well-known for helping manage diabetes and support weight loss, there are growing concerns about their side effects, particularly for people who may be at risk for issues like bone deterioration or inflammatory arthritis.

“People are taking these medications, and obviously there’s a tremendous amount of upside,” he told the publication. “But with that, they start to decrease their intake of food and nutrients.”

Osteoporosis weakens bones, increasing the risk of fractures, even from small falls. This is common for many older adults and those who lose a lot of weight quickly. Gout is a painful type of arthritis that can happen when there is too much uric acid in the body, often due to a diet high in red meat and alcohol, as well as fast weight loss.

The new study (it’s still in the wings, waiting for its peer-reviewed debut) dove into five years of medical records from over 146,000 adults struggling with both obesity and Type 2 diabetes. Researchers closely examined how these individuals fared on GLP-1 medications compared with those who steered clear of the treatments.

While the study shines a light on the potential benefits of GLP-1s, like semaglutide – popularly known as Ozempic and Wegovy – and liraglutide, sold under the names Victoza and Saxenda, it doesn’t specify which drug each patient was on.

Around 4% of people using GLP-1 medications developed osteoporosis, compared to just over 3% of those not using them, which means there’s roughly a 30% higher risk for GLP-1 users. Another bone-health condition, osteomalacia, which causes bones to soften, was uncommon but occurred about twice as often in people taking GLP-1s.

Additionally, the rate of gout was slightly higher among GLP-1 users, at 7.4%, compared to 6.6% for those not using the medication, indicating a roughly 12% increased risk.

“It’s not huge,” Horneff said. “But within that data that was put in there, you even saw nearly a doubling of the risk of having some sort of bone mineral density issue at five years.”

Dr. Clifford Rosen, a professor of medicine at Tufts University who studies the potential impact of GLP-1s on bone health, stresses the importance of more research.

“Weight loss does cause bone loss, but the question we’ve been studying is whether this is a normal compensation of the skeleton, it’s just reshaping itself, or whether this is truly a risk for losing bone that’s more rapid than what would be expected.”

Horneff explained to NBC News that the new research is observational, so it can’t definitively show that the medications caused the conditions. He also pointed out that the researchers didn’t have information about the patients’ diets, exercise routines, or whether they were taking vitamin D or other nutrients essential for bone health.

Despite this, NBC News mentions that the findings are similar to other studies, including one from February in the Journal of Clinical Endocrinology & Metabolism, which found a connection between GLP-1 drugs and an increased risk of osteoporosis-related fractures in older adults with Type 2 diabetes.

So, what are we actually looking at here? GLP-1s are clearly changing the game for weight management, but recent data linking them to sudden spikes in osteoporosis and gout means we can’t just ignore the fine print.

Yes, the science is still evolving and we desperately need more trials that don’t treat Black health like an afterthought. But while the researchers play catch-up, why wait around to become a statistic?

Before you sign up for that first injection, pull up on your doctor with some real questions. Schedule a baseline bone density test, get your joint health evaluated and map out a heavy resistance training routine to preserve your bone mass before the weight starts dropping.
